These forms should be completed and approved before research begins to avoid unnecessary risk but can be turned in during or after data collection is completed. These must be turned in by January 19, 2024 to be considered for the District Science Fair at Blue Peak/Community Learning Center on February 1, 2024.
***Important*** Students/parents should contact Sean Fetherston directly for additional forms if any student is conducting research involving the following:
*Working in an industrial or laboratory setting with a professional scientists or supervisor
*Human Participants
*Vertebrate Animals
*Potentially hazardous biological agents
*Potentially hazardous substances and/or devices
*Potentially hazardous chemicals and/or materials
If students conduct research involving any of the above without prior approval, then they won’t be able to participate in the science fair.
