Please be sure to include signatures and requested documentation.
If you need access to a printer,
please call your school's main office to set up a time.
School fee waivers do not apply to free or reduced meal benefits.

School Fee Waiver Application
- NO fees may be charged for activities during the regular school day in grades K–6. Fees for activities held outside of the regular school day are subject to fee waivers.
- Fees shall be suspended while the fee waiver is under consideration and during the appeal of a denial of a fee waiver.
- Upon approval, school fees shall be waived up to the LEA’s established dollar amount cap, and trip waivers are limited to no more than two trips per student per year.
- A parent/student cannot be required to agree to an installment payment plan or sign an IOU in place of a waiver.
- However, an LEA may provide service alternatives to satisfy a fee requirement.
- A student requesting a waiver for a second trip must complete an action plan that includes school-related contributions.

School Fees Notice for Families of Students in Grades K–6
The Utah Constitution prohibits the charging of fees in elementary schools.
If a child is in kindergarten through grade six, they cannot be charged for textbooks, classroom equipment or supplies, musical instruments, field trips, assemblies, snacks (other than food provided through the School Lunch Program), or for anything else that takes place or is used during the regular school day.
Fees may only be charged for programs offered before or after school, or during school vacations.
Fees may also be charged for a student that is in grade six and attends a school that includes one or more grades 7–12, if the school follows a secondary model of delivering education to the school’s grade six students.
Utah law requires schools that charge fees to ensure that a fee waiver or other provisions in lieu of a fee waiver are available to any student whose parents are unable to pay a fee.
Fee Waivers
A fee waiver is a release from the requirement to pay a fee.
If a student qualifies for fee waivers, fees must be waived, subject to school dollar amount caps on total fee waivers and trip waiver limits.
Fee waivers for school-sponsored trips of two or more nights are limited to two trips per student each year.
A waiver for a second trip may be requested by meeting with the principal or designee to develop a school-approved action plan.
A student is eligible for a waiver if:
- The family receives TANF or SNAP (Supplemental Nutrition Assistance Program)
- The student receives SSI (Supplemental Security Income)
- The student is designated McKinney-Vento
- The student is in foster care
- The student is in state custody
- The student is eligible based on family/household income (same levels as free lunch eligibility)
A student may also be eligible if they do not meet these standards but are still unable to pay. See local school or district policy for details.

- Costs for school pictures, yearbooks, or similar items are not fees and are not waived

School Fees Notice for Families of Students in Grades 7–12
Utah law permits schools to charge fees in grades seven through twelve.
A student may be charged fees for participation in school classes, activities, and programs. A student cannot be requested or required to pay any fee unless that request or requirement has been approved by the local Board of Education and included in the school or district fee schedule.
Utah law requires schools that charge fees to ensure that a fee waiver or other provisions in lieu of a fee waiver is available to any student whose families are unable to pay a fee.
Fee Definition
A fee is defined as something of monetary value that is requested or required by a school for a student to be able to participate in an activity, class, or program that is provided, sponsored, or supported by a school, charter school, or district.
This request or requirement can also be implied. Meaning that if something is not required for participation, but there is still an expectation for the student to have that item, it becomes a school fee.
Examples of fees:
- Truancy fines
- Participation fees
- Remedial programs
- Costs of class or team trips (including room, board, and meals)
- Entrance fees for school concerts or games
- Football summer clinic costs
- After-school program fees
- Instrument rentals
Not considered fees (and do not need to be waived):
- Class rings
- Yearbooks
- School pictures
- Letter jackets
- Similar optional items
If a student loses or damages school property, the costs of replacement or repair are not fees and do not need to be waived.
Students may be required to pay fees for:
- Concurrent enrollment courses
- Advanced placement exams
The portion of these fees related specifically to college or post-secondary credit is not subject to fee waiver. However, the College Board offers reductions for eligible students.
Fee Waivers
A student is eligible for a fee waiver if:
- Their family receives TANF or SNAP
- The student receives SSI (Supplemental Security Income)
- The student is designated McKinney-Vento
- The student is in foster care
- The student is in state custody
- The student qualifies based on family/household income (same levels as free lunch eligibility)
A fee waiver is a release from the requirement to pay a fee.
If a student qualifies:
- Fees must be waived, subject to school caps and trip limits
Trip limits:
- Waivers for school-sponsored trips of 2 or more nights are limited to two trips per year
- A second trip waiver may require a meeting with the principal and development of a school-approved action plan
Note: A student may still be eligible for a fee waiver even if they do not meet the listed criteria but are unable to pay. See local policy for details.

If a student applies for a fee waiver:
- Documentation of eligibility is required
- Submit the School Fee Waiver Application
- Copies are included with this notice or available at the school office or state school fees website
- Always keep a copy
Once documents are submitted:
- Fee requirements are suspended until a decision is made
If denied:
- The school will send a Decision and Appeal Form
- The form explains why and how to appeal
If an appeal is submitted:
- Fees do not need to be paid until the appeal is decided
Donations, Confidentiality, and Enforcement of Fees
- Schools may request tax-deductible donations, but cannot require them
- A student’s name and status regarding fees or waivers is confidential
The school may:
- Recognize donors (with consent)
The school may not:
- Withhold or alter grades or credit
- Withhold class schedules, report cards, transcripts, or diplomas
- Use academic consequences to enforce payment

Parental Appeal Rights
IF YOU DISAGREE WITH THIS DECISION, YOU HAVE THE RIGHT TO APPEAL.
To appeal, send a letter (or the Notice of Appeal form printed at the bottom of this page) to the principal/charter school director explaining why you disagree with this decision. Include your name, your child’s name, and the date.
YOU MUST MAIL OR HAND-DELIVER YOUR APPEAL WITHIN TEN SCHOOL DAYS OF RECEIVING THIS NOTICE.
Keep a copy of the appeal for your records.
A school representative will contact you within two weeks after receiving your appeal and schedule a meeting to discuss your concerns.
You will also be given a copy of the district’s/charter school’s Fee Waiver Appeals Policy containing a complete statement of policies and procedures for appeals.

Utah law states that an LEA may allow a student to perform service in lieu of a fee “to ensure that no student is denied the opportunity to participate in a class or school-sponsored or supported activity because of an inability to pay.”
If offered, a student may choose to perform service in lieu of a fee, but a school district cannot require a student to perform service in lieu of a fee waiver.
However, a student requesting a waiver for a second school-sponsored trip must develop a written action plan with the principal to contribute service to the school or community. This action plan shall:
- Be tailored to the student’s individual circumstances, abilities, and availability
- Be designed to avoid interference with the student’s academic or personal responsibilities
Fee Waiver Limits for Travel
Fee waivers for school-sponsored trips (travel of two or more nights) are limited to a maximum of two trips per student per school year, as required by Utah Code Section 53G-7-504.
This limitation does not apply to travel related to in-state activities sponsored by an association, as defined in Utah Code Section 53G-7-801.
All fee waivers are further subject to the total dollar amount caps adopted by the LEA governing board.
Service Options
If a student chooses to perform service or is completing an action plan for a second trip, service options may include:
- Service at the school (custodial work, tutoring assistance, or student aide work outside school hours)
- Service in the community (such as nursing homes or hospitals)
- Service in the home when special needs are required
- Any other school-related service approved by the principal or designee

Service Requirements and Conditions
The school must ensure that service assignments are:
- Appropriate to the student’s age, physical condition, and maturity
- Consistent with state and federal laws, including the Federal Fair Labor Standards Act
Requirements include:
- Service must be completed within a reasonable period of time
- Each hour of service must be credited at least equal to the current minimum wage
- Daily and weekly service hours must comply with age-based limits
Example:
If a student owes $100 and service is credited at $10 per hour, the student may complete up to 10 hours of service.
Student Protections
Service assignments must:
- Not subject students to embarrassment, ridicule, or humiliation
- Not provide direct private benefit to school employees or their families
- Avoid excessive burdens on students and families
- Consider transportation, education, and personal responsibilities
If circumstances make completion difficult, notify the school principal to determine appropriate adjustments.
