1. Concurrent Enrollment provides an option for prepared high school students to take courses that earn both high school and college credit.
2. Concurrent Enrollment (CE) courses are college courses. College course(s) require rigorous academic work. Course content may include controversial cultural, religious, political, aesthetic and human sexuality issues. Students must have the maturity to engage with the material in an academically appropriate manner and the organization and time management skills to complete more work and meet deadlines. Students must also meet all college/university prerequisite requirements before enrolling in a CE course.
3. The CE program is open to high school juniors and seniors, sophomores may be admitted on a case-by-case basis. Students must be enrolled in a Utah public high school and be counted in the average daily membership. Students who have received a diploma, whose class has graduated from high school, or who have participated in graduation exercises are not eligible to participate in the CE program.
4. Students may attempt up to 30 CE credits per year and may not repeat a CE course.
5. CE course registrations and grades are recorded on permanent college transcripts.
6. Withdrawing from or failing a CE course may impact a student’s ability to receive financial aid in the future as well as a student’s ability to enroll in additional CE courses.
7. SUBMISSION OF THIS FORM DOES NOT ENROLL THE STUDENT IN CE COURSE(S). The student is responsible for completing the admission and enrollment process, paying applicable admission fees, and tuition.
8. If a student withdraws from a course at the high school, s/he must also withdraw from the course at the college. Failure to do so may result in a failing grade on the college transcript.
9. CE credit earned shall be transferable from one USHE institution to another. Students should seek advice from a college academic advisor to make course choices that meet their educational goals and transfer as equivalent credit.
10. CE students must abide by the college/university Student Code of Conduct as well as the high school Student Code of Conduct.
11. CE course offerings are based upon instructor availability, and courses may be cancelled.
12. Students must contact the college or university CE administrator to request any accommodations that may be required under the Americans with Disabilities Act and Section 504 of the Rehabilitation Act.
13. Under FERPA, an institution may disclose to parents information from the education records of a student who is “dependent” under the Federal tax laws without the student’s consent. The education records shared are limited to the courses a student is enrolled in and tuition and/or fees owing for the courses. Have you claimed your student as a dependent in the most recent tax year for Federal tax purposes?

In addition to the State Student Participation form (above) Tooele County School District has put in place the following requirements: (Please read and check off each item.)
□ I understand that I must have concurrent coordinator and counselor approval for concurrent enrollment courses PRIOR TO ENROLLING
□ I understand that I must have concurrent coordinator approval for withdrawal from concurrent enrollment courses.
□ I understand that all Concurrent Enrollment paperwork MUST be filled out online with my legal name – not abbreviations or nicknames.
□ I understand that I must have a cumulative GPA of 3.0 for general education classes such as: Math, English, Science, and History concurrent courses. Students with a cumulative GPA of 2.0 (USU) can take select career and technical education or art classes (this is on an individual student basis, with district administrative approval, with a signed contract and plan to bring up the student’s GPA)
□ I understand that by NOT completing the concurrent admissions application process and registration process, I will not receive college credit or additional high school credit that a concurrent class may be eligible for.
□ I understand that concurrent enrollment courses charge a $5/college credit (subject to legislature) partial tuition up through the first 30 credit hours per year (June to June).
□ I understand that once I graduate, I am no longer eligible for Concurrent Enrollment.
□ I understand that if I receive a “D” or “F” in a CE course, I will not be approved for additional enrollment in CE courses. I must maintain a minimum 2.0 college GPA to be in good standing.

□ I understand that concurrent enrollment classes are college level courses. I am fully aware that these are more difficult courses and that there is a more rigorous pace to the class. A great deal of time must be spent on homework in each concurrent class in order to obtain an “A” or “B”.
□ I understand that Concurrent Enrollment classes may be posted as additional credits on my transcript. One “C” grade in a 3 college credit concurrent class will post as a full year (1 credit) of “C” grades on my High School transcript.
□ I understand that a placement test or ACT score may require for English and Math 1050 courses (see institution for requirements). Math 1030 and Math 1040 require a minimum of a “C” composite grade in Math I, II, and III.
□ I understand that it is my responsibility to order college transcripts from each college I have taken concurrent enrollment classes from in order to be admitted to the college I choose to attend.
□ I understand that as a student enrolled in concurrent courses, I need to be self-disciplined in my behavior management. Some concurrent courses may be taught through IVC/Ednet by university staff and finish before the end of the semester. I understand that if I am enrolled in a concurrent course that this is the case, I shall be released to my parents’ responsibility.
□ I understand that colleges do not accept any late forms or late work. Meeting the deadline is the first college test I MUST pass.
□ I understand that college level grading standards will be used, and I will comply with the standards set forth in the course syllabus. I also understand that the grade(s) I receive for courses will become a permanent record on my college transcript, regardless of the results. The final grade that appears on my high school transcript must be identical to the final grade that appears on the college transcript.
